The purpose of this Request for Proposals (RFP) is to solicit proposals to enter into a contract with a qualified Vendor to obtain Leased Lit Fiber Service or Leased Dark Fiber/IRU Service for the Utah Education and Telehealth Network (UETN) of the University of Utah. The award of this solicitation may result in multiple awards. Vendors responding to this solicitation must be currently registered with the SLD and must provide the Vendor Identification Number (SPIN)/FCC 498 ID as assigned by the SLD within the response to this solicitation. Vendors must also be current in their Form 473 (Service Provider Annual Certification - SPAC) and maintain current certification throughout the length of an awarded contract period. The anticipated effective date of the Contract(s) is July 1, 2022. Vendors must not proceed on any work orders until UETN provides a written Notice to Proceed.
