Description
Contractor shall collect, remove, transport, and legally dispose of all environmentally-regulated,
biological and hazardous waste, and contaminated materials, debris, waste and other substances
(collectively the "HazWaste Materials") at the sites set forth in the Service Request, and also cleanup
the sites so that no HazWaste Materials remain on the site (collectively the "HazWaste Services").
The HazWaste Materials include, but are not limited to, contaminated debris, biological waste,
environmentally regulated substances, and hazardous waste, including the HazWaste Materials in
confined spaces. Biological waste is any material that contains or has been contaminated by a
biohazardous agent (risk to human health or the environment), and it includes, but it is not limited to,
syringes, needles, blood, vomit and feces
1. Bidder must have, at the time of quote submittal and for the duration of the Agreement, the following current, valid licenses, registration, and certification:
A. Classification HAZ Hazardous Substance Removal Certification from California Contractor¿s State License Board (CSLB).
B. Hazardous Materials Certificate of Registration issued by U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration.
C. Hazardous Materials Transportation License issued by California Highway Patrol (CHP).
D. Trauma Scene Waste Management Practitioner (from State of California Department of Public Health, Medical Waste Management Program).
E. Waste Tire Hauler Registration issued by Cal Recycle.
F. Hazardous Waste Operations and Emergency Response (HAZWOPER) Certificate of 40 hours of training and current 8-hour refresher course certificate.
G. Motor Carrier Permit issued by California Department of Motor Vehicles (DMV).
H. Hazardous Waste Transporter Registration issued by State of California-California Environmental Protection Agency (CalEPA), Department of Toxic Substances Control, Health and Safety Code-Division 20. https://www.dtsc.ca.gov/HazardousWaste/Transporters.cfm
I. Confined Space Entry Training Certification as required by the Department of Industrial Relations, Division of Occupational Safety and Health (Cal/OSHA), in Exhibit A, Section 19.
J. Pest Control Business (PCB) license and a Qualified Applicator License (QAL) with Category K (health related) issued by Department of Pesticide Regulation.
K. Bidder must be registered with the Department of Industrial Relation (DIR) pursuant to Labor Code Section 1725.5 Bonds required. Prevailing wages apply. DBE incentives. Term is through 9/14/24. Counties covered under this contract: Sacramento, El Dorado, Placer, and Yolo.
