Description
Contractor’s Full Business Name (to be completed by the DMV), hereinafter referred to as the
Contractor, agrees to provide the Department of Motor Vehicles (DMV) all labor, tools, materials,
equipment, and travel necessary to perform heating, ventilation, and air conditioning (HVAC) duct
system maintenance services at the Van Nuys DMV field office for the term of this Agreement. The
Contractor is responsible for field verifying all dimensions and existing site conditions pertinent to this
Agreement. Bonds required. Prevailing wages apply. Term is two calendar days within the period of 10/1/21 through 6/30/23. Questions due by 3pm on 10/13/2021. DBE incentives.
