SCOPE OF SERVICES shall include, at a minimum, the following elements: A. INITIAL INSPECTION Contractor is to provide a detailed inspection protocol for assessing and identifying in structural damage or conditions (water intrusion, window damage, leaks, etc.) that must be addressed to thoroughly remediate existing mold and to prevent its reoccurrence. This is to include bulk material samples and interior wall cavity samples where and when necessary. The results of such testing should be immediate or within such a time period as to not unnecessarily delay informed decisions regarding treatment procedures. B. CLEARING AND/OR REMOVING DEBRIS AND WASTE Contractor shall assess the interior of the designated Board facility initiate for service and remove or have removed any debris that inhibits or contributes to the damage of that facility or impedes the contractors efforts to begin servicing the facility. All debris shall be disposed of in accordance with the current required standards. C. DOCUMENTATION MANAGEMENT AND SUPPORT The Contractor shall assist the School District in preparation of FEMA, State, and private agency reports for eligible reimbursement, including training of School/Department employees and review of documentation prior to submittal. The contractor should provide in the description of services how you track and account for employee time and expendables used in disaster recovery incident(s). The Contractor shall work closely with State and County Emergency Management, FEMA, and other agencies to ensure that all supporting data and documentation meet each agencys requirements for reimbursement eligibility. D. OTHER EMERGENCY RESPONSE & DISASTER RECOVERY SERVICES 1. The Contractor shall provide their typical program approach to Fire, Flood or Disaster Restoration, stating the responsibilities of each of the key participants (such as): a. Project Manager responsibility, control and on site decision making authority. b. Site Environmental and Safety Officer responsibility for the health and safety, authority (who he reports to), etc. 2. At minimum, the scope of services required to be provided by the selected firm or firms are: To provide Fire, Flood, or Disaster Restoration services for all emergency and long term final repairs. Document and provide on-going periodic written/electronic reports, including humidity readings, etc. j. Host meetings with MCSD to communicate work progress, etc. k. Provide to MCSD, at completion of work, a written/electronic comprehensive final report with detailed information that summarizes all covered services provided by the contractor and in sufficient detail to satisfy FEMA and insurance company requirements in order to maximize payment of claims and reimbursement of expenses. l. 1. Bid/Contract prices shall include all scheduled inspections and maintenance needed to complete service. This includes routine maintenance of all County maintained Chillers and their associated components. Should Monroe County require additional services or items provided by Contractor but said services and/or items have not been specifically defined in the Bid/Contract Documents, then a proposal will be requested from the Contractor and, if the proposal for these services and/or items is acceptable to Monroe County, a separate purchase order will be obtained for payment thereof. In all other respects, the contract parameters will be adhered to by the Contractor. 2. Corrective Maintenance - Necessary repairs for deficient or inoperable devices, such as those found during the course of system testing, inspection, or preventative maintenance, or deficient or inoperable devices that have failed during operation, will be provided by the Contractor. Only original replacement components manufactured by the original equipment manufacturer or other compatible components are to be used for such repairs. 3. Ductwork and Piping Insulation repair and replacement - Maintenance and Repair of the chillers, pumps, and cooling towers systems shall include Ductwork and Piping Insulation inspection, repair, and replacement work. Insulating piping and ductwork throughout Monroe County buildings shall be conducted on an ongoing basis as needed. 4. The Contractor shall be available twenty-four (24) hours per day, three hundred sixty-five (365) days per year. The Contractor shall be at the site of any Chiller system malfunction within four (4) hours of verbal, written, or text notification by the County. The Contractor shall provide an after-hours contact person and phone number. The County, upon award of the contract, shall provide a contact person and phone number for building and equipment access. 5. The Contractor shall have access to a supply of all parts and controls normally necessary for the emergency repairs of all County chillers so that such emergency repair will be completed within forty-eight (48) hours of notification by the Owner. 6. The Contractor will be responsible for obtaining all necessary permits and approvals for the work including: Federal Aviation Administration, Historic Architectural Review Commission of the City of Key West, City of Key West Page 92 of 111 Building Department, Monroe County Building Department, City of Marathon Building Department, Villages of Islamorada Building Department, and any other permitting or regulatory agencies as applicable. 7. The Contractor shall not store materials, tools, or debris on the premises without written permission. The Contractor shall take all means necessary to contain dust and debris as an integral part of the work. The Contractor shall provide a suitable storage container and be responsible for disposal off-site of all debris and trash. 8. The Contractor needs to be aware of the facilities, its vendors, and staff with unusual schedules and plan accordingly. Coordination of each days work with the County staff contact shall be done in advance. All spaces, interior and exterior, shall be cleaned and returned to normal each workday
